Amy offers many alternatives when paying for her services.
Fee for Service is available
- $200 for initial evaluation
- $150 for 1 hour session
Amy is in-network with Blue Cross Blue Shield PPO
- Client must ascertain their limits and be prepared to pay their deductible and/or copays at the time of visit. Amy will bill BCBS directly on your behalf.
For out of network insurance plans:
- If your Insurance is not BCBS PPO, then I would be considered an “Out of Network Provider”. I will collect your payment at the time of service and supply you with a Superbill. A superbill has all the information that your Insurance company will need in order to reimburse you at an “out of network” rate.
Amy accepts cash, credit card, debit card, and health savings account (HSA) cards.
